To register your child in the Elsberry R-2 School District, you will need to complete the Student Enrollment Forms. You may view and print the ESD New Student Enrollment Packet. If you are registering more than one child, an Additional Child/Children enrollment packet must be filled out for each child.

Before submitting Enrollment Packets, be sure to include the following information:

• copy of student's birth certificate

• copy of student's social security card (requested but not required)

• copy of immunization records

• proof of residency

• oath of discipline

Please submit the enrollment packet and additional documentation to the ESD Registrar's Office. For questions regarding the enrollment process, please contact the Registrar's Office at 573-898-5554 x225.

If you think you may qualify for Free or Reduced Lunch prices, please complete the application and return it to the registrar's office.
